Preop Checklist

  • Preauthorisation V4080, fusion and uplift forms

  • Provisional Op Date

  • Consent appointment within 2-4 weeks

  • MRI (Lumbar spine within one year prior)

  • Planning Films: Long Spine standing AP/ Lateral, AP side bending standing lumbar, Lateral Lumbar centred over level planned to operate

  • Theatre booking

  • Amplitude / Spine registry entry

Operation Checklist

  • IOM- usually bespoke- ask them to bring a probe,

  • Bloods: G&S, FBC, UE, LFT, Coag, Vit D, Bone profile

  • Consent for operation

  • Allen or OSI table with Flat top, 3x side clamps

  • Arm supports x2 from maquet table

  • Sheet, 2x towels rolled

  • Axillary roll, 2x pillows for head, 2x pillows for between knees

  • Wide tape

  • 2 Bair huggers

  • Diathermy with smoke evacuator set to Mono Cut 50 Coag 50, Bipolar 36

  • Imaging available: Standing XR AP & Lateral, MRI L spine, Bending films

  • Image intensifier- call for when tube down

  • Catheter after antibiotics

  • Chloraprep resistant marker pen

  • Alcoholic betadine, chloraprep x2 (do not crack)

  • Large drapes (do not cut in half) to square off. Large side drape for lateral II or preferably C-Armor.

  • Ioban seal

  • Blades/handles: Skin: 10/std, Disc: 15/long

  • Ravine Retractor set

  • Cascadia 3d printed cages: 8 degrees, 8, 10 , 12 mm height, 50, 55, 60 mm length, 22mm width

  • Vitoss BA 10cc strips x3 to be available- open one at a time

  • 1 PDS, 2/0 PDS, 3/0 monocryl

  • Liquiband or cyanoacrylate glue 

  • Surgistrips,

  • Mepore Film&Pad
